 It is REQUIRED you meet with the Childcare Director at Edgemoor Community Center BEFORE turning in the registration forms. Please see the Childcare Registration Steps page for the list of steps for applying. Thank you!

Once you have met with the Childcare Director, download and complete the following information and then bring it to the Edgemoor Community Center Main Office (please do not email forms).

If your child does not have a current health appraisal on file with ECC (less than two years), please download and complete the following form:
